I walk daily, have a few old injuries I'm nursing, and have a wide foot, narrow heel, high arch and high instep. My foot measures a tad over size 10 W and I bought two pairs of the 11 W. My toes have lots of room. No pain at all. So happy.

The toe-box is a little wider than a Brooks shoe, which allows my forefoot the needed room to swell slightly, as will happen on a long run. The heel still fits snugly, and does not slide. The cushoining is ample, and my knees really appreciate it! I have now worn this shoe in 2 marathons, and for many many many training runs, and have LOVED it. It's the shoe I reach for first.
