I had originally bought these basing my size calculations on the website's Scarpa shoe size info and the pair I got were tight all around. They seem to use a narrower last (Italian company, Italian feet) but the length of the shoe had my toes right at the ends--just touching the inside. By the end of the day my feet felt like they'd been in a tourniquet.
I went to a half size larger and these give me the needed space in the toe box for all day comfort. I wear a an honest 11.5 U.S. The 45.5 size in this shoe (11 2/3 US) were the tight ones. The 46 (12 US) fit just fine.
Great tread, nice breathable comfort, handsome good looks.

    I ordere a size 10 and 10.5 and neither pair fit . They were to narrow in toe box.
