It's fully lined inside with the gingham material you see on the rolled cuffs of the sleeves- so much so that I initially thought the shirt was reversible (it's not). The full lining gives it a nice heavy weight, and the pin tucks at the front were a nice touch that you couldn't really see in the photos. I liked this shirt so much that I went back online and bought it in the blue. I bought a large, and the only thing I will say is that the sleeves are tight when rolled up for the cuffed effect as in the main photo- it felt a little like a muscle shirt, but not so much that it was detrimental, just worth noting. I'm relatively skinny, so if your arms are on the chunkier side, you might not be able to roll up the sleeves as depicted with the cuffs/buttons.
Overall, this shirt is a winner. -

It was also completely lined with a second layer of fabric and the layers didn't seem to be compatible. The size I chose was too small and I had to return it.
