I basically wear a 10D in most dress shoes, though that sometimes increases/decreases by a half size depending on the manufacturer. At first, I ordered a 42 because I was advised to order down, but that turned out to be way too snug. I then exchanged it for a 43, but somehow that managed to be too loose in the heel area on my left foot. I ultimately returned it because it just didn't feel secure enough. The shoe itself looks and feels like it's of good quality, though I was not a huge fan of the ridge at the tip of the shoe. I didn't notice it in the picture, but it bothered me once I saw them and contributed to my decision to return them.
Interestingly though, I subsequently ordered a Blackstone KBM10 in a 43 and the shoe fits very nicely.

He wears a size 10.5 M in Johnston and Murphy and the fit is perfect. We couldn't figure out what it is about this shoe that makes them so loose. The style is very nice, the quality also seems very good, but if the shoe doesn't fit right it's a waste of money. I would suggest you most definitely try it and walk in them at home or at a store first. Unfortunately they didn't work for us.

He wears a 10.5 in Johnston and Murphy and in these the 45 was huge and the 44 too loose. We are going to try the 43, but won't know till Christmas. The quality looks great in that they feel soft and comfortable.
