Manufacture’s error in sizing?

First of all, I am a big fan of Johnstons of Elgin cashmere sweaters – they look great, feel even better, wear wonderfully, last many years, and are true to size (no vanity sizing, how nice!). Thanks to Sierra’s great offerings, I’ve been expanding my collection every year - these sweaters, despite many wears and many washes, don’t wear off at the pace others do.
A few days ago, I ordered this crewneck in a pistachio color in size S. A lovely, vibrant green-colored sweater arrived in a matter of a few days. Unlike my other JofE orders from Sierra, this sweater was shoved unmanageably into a plastic bag, instead of being folded neatly the way other JofE sweaters have always been. OK, this wouldn't be even worth mentioning if not for other things. The size seemed to be off – at 5’4” and 100 lb, I can wear both XS and S from JohE and look fine but anything bigger makes me lost in it. I looked at the attached to the neckline paper tag – it has “S” written on it, I looked at the tag stitched to the left side and “M” is there. What gives? I called Sierra, explained my problem and placed another order. About half an hour later I received a call informing me that my order was cancelled because the remaining sweaters supposedly have the same sizing problem. Yet, more than 24 hours later, I still see these sweaters in a pistachio color in size S listed as available and ready to be purchased. Why weren’t they removed from Sierra’s listing if they are size M? This is one of very few times when shopping at Sierra that I am perplexed.
