Probably could have tried a 41, but it would have been the third pair. Kind of frustrating because my Nepal Evos are a 42.and I thought it would be a similar fit.

I don't use them for mountaineering, but for hunting chukar in steep, loose, technical, rocky terrain. The soles are narrow - essential for finessed foot placement - but fit my weird-shaped foot: narrow in the heel and very wide in the forefoot. I was worried they'd be too narrow up front, as my Asolos were. Somehow the last on this boot is better suited to my foot than any other I've tried. They're comfy right out of the box. I also have a bum left ankle and was worried they wouldn't offer enough support for the ankle, especially since they're two inches shorter than my previous boots, but they're rock solid and the lateral stiffness really keeps that ankle from rolling. In short, these are amazing, and a great deal from Sierra.

They have plenty of support under foot and the support goes up the ankle. I have worn other mountain boots and these are by far the lightest. They offer good traction over loose rock. I don't climb mountains in them, but rather hike over rough trails and they perform wonderfully.
Perfect Mountaineering "Light" BootVerified Buyer Reviewed by Myt1 from Arizona on Saturday, June 24, 2017In order to save weight in my backpack I was looking for mountaineering boots that I could hike in and not have to carry and only wear during summit day.
These boots have a little give in them and I'm able to walk long distances without issues. They needed minimal break-in.
The rear welt allows for wearing semi-automatic crampons.
Size seems to be pretty true. I normally wear size 11 and size 45 metric size fits just fine. This is the size I normally wear when I wear other metric sized boots, or shoes. My foot width is pretty normal to very slightly narrower than normal.
Quality is first rate and they are very easy to take on and off and very easy to lace.
Also, the laces seem to be high quality. So many times boot makers cheap-out and put inexpensive laces on their boots that need to be replaced; that doesn't seem to be the case here.
I will be using these boots for an eight mile approach hike carrying a heavy pack and on summit days I will use crampons and ice axes to climb a moderately angled glaciers followed by class 3 and class 4 rock climbing. Every indication is that these boots will be perfect for this. In fact, they are probably capable of much more technical climbing than this as well.
Keep in mind though they are not insulated.

This model garnered a lot of awards when it first cam out nearly a decade ago, and I should have just bought them then. I bought the Scarpa Charmoz Pro GTX and thought they fit alright, but found they had too much room in the forefoot and (especially) the heel. As soon as I put the Trangos on I knew they were a much better last for my foot—12.5+ C. I went with the 46.5 and it fits perfectly, without heal lift or excess room in the toe box, though you may want to size up a fraction of a Euro size if you wear huge socks. I'd call them true-to-size.
